No, urushiol intoxication does not return periodically. So, what happened in yoru case? I suspect one of two things: 1) you cam in touch with poison oak again. Cure: look at the photos on this site and remember poison oak can be VERY variable in its forms. 2) You used an item of clothing (shit, shoes!) or camping gear (backpack, sleeping bag...) that became contaminated last year, without removing the urushiol oil. You zapped yourself again.
You need to launder / wash EVERYTHING you suspect of becoming contaminated last year. Use lots of soap and water- especially any soap that is a strong degreaser or oil solvent. If it's a solid item, try Formula 409 spray, which is also a powerful degreaser.
